当尼奥提出墨菲斯和崔妮蒂以前都问过的问题时，他做出了一个代价很大的决定，他要寻找并接受真相，将他的灵魂从母体中释放。
一切都从“选择”开始。
现在，当人类最后的城市锡安陷入机器军队的包围时，尼奥充分发挥了他的超能力。短短几个小时要把人类最后的领土从25,000个专门摧毁人类的电脑哨兵手中解救出来。不过，墨菲斯的信念鼓舞了锡安的人们，他们相信“救世主”尼奥将实现先知的预言，结束与机器们的战争。他们把所有的希望都寄托在尼奥的身上。而尼奥在寻找行动的方向时，发现自己陷入令人烦恼的幻觉之中。
为爱情和信念所激励，尼奥和崔妮蒂选择了与墨菲斯一起回到母体，运用他们非凡的技巧和装备，对抗电脑系统的压制和利用。然而，母体内部存在着强大的力量反对他们的自由抉择。
与此同时存在的还有被母体流放的特工史密斯，因为与尼奥解不开的联系迫使他违背系统，导致他被系统删除。一度被他轻视的“人性”驱使他向尼奥进行无穷无尽的复仇。
在进一步了解母体结构的危险旅途中，作为人类命运的关键人物，尼奥将遭遇更大的对抗，面对他无论如何都想象不到的，更惊人的真相和更加困难的选择。
在爱情与真相、信念与知识、目的和动机交错之中，尼奥必须顺着自己选择的方向走下去。“如果我无能为力？”他问，“如果我失败了呢？”

剧情：
尼奥在母体中苏醒过来，此时他正失陷于母体中，身体与思想处于分离状态。他在火车站遇到神秘流浪汉，随后一同搭乘火车。在火车上流浪汗详细告知了尼奥救世主身份的来龙去脉。与此同时，锡安的居民们正在与机器做殊死的搏斗。冒进的战略思想导致他们首轮既遭重创。战后会议之后，墨菲斯和崔妮蒂寻找到了失踪的奈尔碧，并得知了尼奥的情形。
他们一起返回锡安，墨菲斯等人带着尼奥的身体重新进入通讯层去见先知。先知告知他们必须通过地狱，才能从公爵手里救出尼奥。同时还暗示了帕瑟芬与公爵同在一处，并安排了他的保镖六翼天使协同作战。 锡安的情况已经十分危急。两个小时之后，机器的主力部队即将兵临城下。
在地狱深处，前去营救尼奥的一乾人等，遇到了老对手双子星。一场恶战，六翼天使阵亡。在帕瑟芬的帮助下，成功除掉了公爵。但是锡安的情形却没有好转。尼奥获救后，立即履行救世主的义务，除掉作恶多端的双子星。与先知的会面，他清楚了人类与机器已经维系在一起。而特工史密斯则是系统最大的威胁。
崔妮蒂在随后的战役中不幸丧生，如此复杂混乱的局面，先知也无法预言最后的结局。系统本身更是矛盾重重，尼奥只能依靠自己的判断决定人类与机器的命运。
